Deciphering whether a febrile child in the emergency department (ED) has a bacterial or viral infection based on observed clinical symptoms remains a diagnostic challenge. The objective of this investigation is to discover a novel combination of host protein biomarkers and to analyze its utility in discriminating between bacterial and viral infections in febrile children presenting to emergency departments.
A literature review encompassing the period between May 2015 and May 2019 was performed to identify blood protein biomarkers capable of differentiating between bacterial and viral infections. From among the various protein biomarkers, seven were selected: procalcitonin, TNF-related apoptosis-inducing ligand (TRAIL), interleukin-4, interleukin-6, interferon-gamma-induced protein-10 (CXCL-10), interferon-gamma, and lipocalin-2 (LCN2). In the Netherlands, blood plasma samples from children with confirmed bacterial or viral infections presenting at EDs were assessed using a bead-based immunoassay. To classify bacterial and viral infections, generalized linear modeling was utilized, and a previously designed feature selection algorithm was applied for the optimal protein combination selection. We carried out a subgroup analysis examining this protein signature, focusing on patients with C-reactive protein measurements less than 60mg/L, a population presenting a difficult diagnostic picture.
A total of 102 children were enrolled in the study, encompassing 67 cases of bacterial infection and 35 cases of viral infection. The individual contribution of each of the 7 biomarkers in classifying infections caused by bacteria versus viruses showed a receiver operating characteristic curve (ROC) area under the curve (AUC) spanning from 6.08 to 7.45. Further analysis revealed that TRAIL, LCN2, and IL-6 formed the best three-protein signature, achieving an AUC of 86% (95% CI 713%-100%). The 3-protein signature, in a sample of 57 patients with C-reactive protein levels less than 60 mg/L, showed an area under the curve (AUC) of 851% (confidence interval 95% CI: 753%-949%).

However, faculty and learning technologists' specialized skills in digital tool selection, curation, and deployment are frequently underutilized because of uneven faculty-LT relationships and a lack of effective collaboration. Herein, we explain the application of the co-production model to develop equitable and collaborative partnerships among faculty members and learning technologists, leading to the optimized use of digital opportunities and an improvement in online learning.

A method for the efficient synthesis of benzo[c]phenanthridine alkaloids is described, employing a synergistic strategy that combines C-C bond formation with a cycloaromatization reaction. Benzo[c]phenanthridine derivatives, a class of compounds with potential pharmaceutical applications, are synthesized through the reaction of aryl nitrones with 7-azabenzonorbornadienes catalyzed by Rh(III).
